Jail & Reentry Reach-In Services

Starting treatment before release — because that gap in care can be deadly.

What Reach-In Services Are

Reach-in services mean exactly what the name says: rather than waiting for someone to seek treatment after release, we bring evaluation, treatment planning, and — when appropriate — medication-assisted treatment directly to individuals while they're still incarcerated. This dramatically reduces the dangerous gap between release and access to care.

Why This Window Is So High-Risk

The first days and weeks after release from incarceration are one of the highest-risk periods for overdose, particularly for individuals with opioid use disorder whose tolerance has decreased during incarceration.

Without treatment and support already in place, this window is when people are most likely to relapse into use at their prior tolerance level — with fatal consequences.

Reach-in services directly address this by establishing care before the gap ever opens.

What's Included

  • Pre-release evaluation and treatment planning
  • Initiation of medication-assisted treatment prior to release when clinically appropriate
  • Coordination with facility medical and reentry staff
  • A confirmed appointment and continuity plan for the days immediately following release
  • Connection to counseling, primary care, and psychiatric services as part of the same program

What Happens After Release

The goal of reach-in work is a seamless transition — someone leaving incarceration already has a relationship with their care team, an appointment already scheduled, and, if applicable, medication already in place, rather than starting from zero and navigating a new system alone during one of the most vulnerable periods of reentry.

How We Work With Facilities

  • Direct coordination with facility medical staff and reentry coordinators
  • Scheduling built around release dates and facility requirements
  • Documentation that satisfies both clinical and correctional recordkeeping needs

Frequently Asked Questions

How does someone get connected to reach-in services while incarcerated?

This typically happens through coordination with facility medical or reentry staff — reach out to our office and we can help facilitate that connection.

Can medication-assisted treatment really start before release?

Yes, when clinically appropriate — this is one of the most effective tools for reducing overdose risk in the immediate post-release period.

What if someone doesn't have an appointment set up before their release date?

Reach out to our office as soon as possible — we prioritize getting people connected to care quickly, even without advance notice, given how high-risk this transition period is.

The gap between release and treatment is where lives are lost. Reach-in services exist specifically to close that gap before it opens.
