About Us
For Referring Providers
One coordinated referral, across medical, behavioral health, and rehabilitation care.
Why Refer to Mojave
Patients rarely fit neatly into a single specialty. A patient with chronic pain may also be managing anxiety or a substance use history. A patient recovering from a car accident may need medical evaluation, physical therapy, and trauma-informed counseling together, not three separate referrals to three disconnected practices. Mojave was built specifically to give referring providers, attorneys, case managers, and courts a single point of contact for patients who need more than one type of care.
When you refer a patient to us, you're not handing them off to a black box — you're connecting them to a coordinated team that keeps you informed and works within the scope of whatever brought the patient in.

What We Treat
Our practice spans substance abuse treatment (including medication-assisted treatment), behavioral health and psychiatry, primary care, physical therapy — including vestibular and neuro rehabilitation — personal injury care, workers' compensation care, and justice services. A referred patient can be evaluated across whichever of these areas applies, coordinated as one plan rather than separate intake processes.
How to Refer a Patient
Referrals can be made by phone or fax, or through direct provider-to-provider contact. We aim to schedule new patient evaluations promptly and will confirm receipt of your referral and follow up on scheduling.
What Referring Providers Can Expect
Prompt evaluation and scheduling for referred patients
Direct communication back to you on evaluation findings and treatment status, with appropriate patient consent
Coordinated treatment across every department the patient's situation actually requires
Documentation built to support your specific need — medical, legal, or compliance-related
Frequently Asked Questions
Can I refer a patient for just one service, like psychiatric medication management?
Yes — patients can be referred for a single service, and we'll expand their care plan only if a fuller evaluation identifies additional needs, always in communication with you.
How do you handle communication back to the referring provider or agency?
With appropriate patient consent, we provide updates on evaluation results, treatment status, and progress at a cadence appropriate to your role — a treating physician, an attorney building a case, and a supervising probation officer each have different information needs, and we tailor communication accordingly.
Do you accept referrals for patients involved in litigation or workers' compensation claims?
Yes — we regularly work with attorneys, employers, and claims adjusters, and our documentation is built to support those processes directly.
A good referral relationship means your patient actually gets seen, actually gets a coordinated plan, and you actually stay in the loop. That's what we aim to provide every time.
