Mojave serves the East Valley from one clinic at 1010 E McDowell Rd, Suite 410 in central Phoenix. Primary care, behavioral health, physical therapy, injury care, and addiction medicine are part of the same outpatient practice.

Not a physical branch — care is provided at our central Phoenix clinic at 1010 E McDowell Rd, Suite 410, a 15 to 50 minute drive from most East Valley cities via US-60, Loop 202, or I-10. Telehealth counseling and consultations are also available anywhere in Arizona.
Is the drive worth it compared to a closer single-service clinic?
Most patients tell us yes. Because medication, counseling, psychiatry, and medical care happen in one visit at one location, you make fewer total trips than you would juggling separate providers closer to home.
Can I come to you after a car accident or a work injury?
Yes. After an accident you can schedule directly, with no referral or attorney needed first; if you have an attorney, we'll coordinate with their office once you're a patient. For work injuries, we send regular work-status updates to your employer and claims adjuster.
